All appointments require a referral form from your physician, except a screening mammogram. Please bring the referral form with you to your appointment, if not sent by your physician's office. Appointment availability depends upon the nature and urgency of the medical problem. We will make every effort to accommodate your preferred date and time.
There are several ways to make an appointment:

Monday through Friday – 7:30 am to 5:30 pm
- Your physician’s office may call our scheduling department
to make your appointment. At that time, the physician’s office
will fax your referral slip to us.
- Come in to our office at Silverdale (1780 NW Myhre Road, #1220)
or Bremerton (2601 Cherry Avenue, #105) and schedule your appointment at the Reception Desk. You will need to bring your referral form with you
when scheduling your appointment.

Our scheduling department will make every effort to process your request as quickly as possible, usually within 24 hours. They will call you the following business day.
